Iwao Asakura, a.k.a. Gen, president of Asakura Corporation, a subcontractor demolition company, regains his passion as a world-class demolition engineer when Keiko Otsuki, an employee of major general contractor Sanyu Heavy Machinery Lease, brings him a demolition job. He accepted her invitation to join the \"Sanyu Group\" and established the company \"Sanyu Blasting Co. Later, he added an old acquaintance, Roku Iwashita a.k.a. \"Roku the Tugger\", a wooden construction specialist, Hideo Tokita a.k.a. Hide, a crane engineer who was about to commit suicide due to debt, and Hikaru Nakahara, a truck driver and former TV personality, to his team. However, in order to become \"Japan\'s No. 1 demolition company\" that is not bound by the traditions of bid-rigging that persist in the construction industry, he leaves the Sanyu Group and reestablishes himself as Itsumo Blowup Co.

Their powers have been used in the underworld for covering up accidents, assassinations, and all manner of crimes. The story involves human mutations called "Diclonius", who grow horns on their heads and possess telekinetic powers, in the form of invisible "arms" called vectors. The tendency for violence among Diclonii varies among characters. Certain scientists contain them in a Facility in cells, where they are subject to inhumane experiments to test their abilities or where they await execution for being too dangerous. An infectious virus is also associated with the mutation. This virus is spread through non-lethal contact with the vectors of a diclonius. The main non-mutant character in the story is Kouta, who ends up becoming increasingly involved with these dicloniuses, as they seek refuge from the scientists. The main diclonius character is Lucy/Nyuu, a killer whose alter ego is a hapless and very cute child whose vocabulary and intelligence are quite limited.
The story revolves around three teenagers: Nomiya Tomomi, a high school dropout, Togawa Kiyoharu, an ex-sprinter who now plays wheelchair basketball and Takahashi Hisanobu, a popular leader of the high school's basketball team who now finds himself a paraplegic after an accident. Real features a cast of characters who find themselves being marginalized by society, but are all united by one common feature: a desire to play basketball, with no place to play it in. Nomiya, being a high school dropout, has no future in his life. Togawa, being a difficult personality, finds himself constantly feuding with his own teammates. Takahashi, once a popular team leader, now finds himself being unable to move from the chest down. Real also deals with the reality of physical disabilities, and the psychological inferiority that the characters struggle against. The characters break through their own psychological barriers bit by bit.
